Payer Information
Freedom’s home care services are covered by Medicaid waiver programs (HCB, Michelle P., and SCL waivers in Kentucky; PASSPORT waiver in Ohio; Community HealthChoices in Pennsylvania; CHOICES program in Tennessee), select managed care organizations, VA benefits, long-term care insurance, and private pay. Our intake team verifies eligibility and handles payer coordination at no cost.
Adult Day Healthcare

Freedom operates 8 adult day and IDD centers in Kentucky, with planned expansion in Ohio, Pennsylvania, and Tennessee. Our centers combine skilled nursing oversight with culturally responsive enrichment programming. Centers are protected by Certificate of Need (CON) requirements that limit new market entrants.
