Health Plans & Agencies

A partner for the work behind care

Health plans, Area Agencies on Aging, VA teams and community partners need to know what is available, how referrals move and who follows through. Freedom combines local care teams with shared workforce, service-documentation and compliance resources. We can discuss scope, escalation contacts and reporting for a specific program and market.

What we bring to the partnership

  • Local teams.

Staff who understand the communities they serve and the realities of arranging support close to home.

  • Workforce accountability.

For agency home care, caregivers we employ are W-2 employees, with screening, training and supervision appropriate to their roles.

  • Service documentation.

Electronic visit verification is used where the service and program require it.

  • Compliance leadership.

Our compliance function supports questions, review and escalation through approved channels.

Participation and accountability depend on the market

Network participation, covered services, reporting and quality expectations are specific to the contracting entity, service and geography. We confirm those details before making commitments. For an agreed partnership, we can define how referrals, authorizations, service starts, missed visits, documentation exceptions and escalations will be handled and reviewed.

Where access is difficult

Tell us about a county, language community, service gap or population that is hard to place. We can review the local need, available workforce and service model with the appropriate market team. Useful measures may include days to first service, unserved days, continuity and referral closure, with definitions and reporting responsibilities agreed before work begins.

Exploring a pilot

For a focused initiative, we can jointly define the population, geography, responsibilities, baseline, practical measures and review schedule before committing to a pilot. Scope and timing are agreed in writing, based on capacity and program requirements.

What we can review together

Depending on the service, program and available systems, partners may discuss timely starts, caregiver continuity, missed visits, electronic visit verification exceptions where required, incident follow-up and referral closure. Measures are defined together. An EVV match rate is not a stand-in for care quality.
