Adult Day Health
A day that still feels like yours.
Our centers give adults a welcoming place to spend the day among people who get to know them. The day can include shared meals, activities they choose, conversation and health support based on the center and the care plan. Participants keep living at home, and family caregivers get dependable daytime support.

For people who want more from the day
Adult day health can fit someone who lives at home and would benefit from company, structure and support during the day. It can also help family caregivers who work or need reliable daytime support. Many participants and families simply call it the center.
What happens at the center
Familiar faces and activities
Games, music, exercise, crafts, outings and conversation, with each center shaping the day around the people who attend.
Meals together
Breakfast or lunch and snacks, with familiar foods where we can.
Health support during the day
What health support looks like depends on the center and the care plan. Ask your coordinator what the center nearest you offers during the day.
Getting to the center
Transportation availability varies by center and program. A coordinator will explain what applies at the location nearest you.
The center becomes part of your community.
Our centers grow out of the neighborhoods around them. That shows up in the food, the music, the activities, the languages in the room and the simple fact of walking in and hearing your name.

Some families use both.
Home care and attendant care bring help into the house. Adult day health adds daytime company and another layer of support. Many Freedom families use both. Your coordinator can explain what fits the person’s needs, program rules and local availability.
How adult day health gets paid for
Medicaid waiver programs may cover adult day health for eligible participants where Freedom participates. The VA may authorize adult day health for eligible veterans where Freedom is an approved provider. Private pay is available at some centers. A coordinator can explain the pathways available at the center nearest you. The program or payer decides who qualifies and what it covers.
