For case managers, health plans and referral partners

Refer with confidence.

Freedom Senior Services gives case managers, health plans and other referral partners a clear handoff and a named contact. When we receive a referral, we review the service area and care need, connect with the family, work through the applicable requirements and keep you informed about what comes next.

Make a Referral Two figures reach toward each other across an orange heart; a flowing line carries the connection to the Freedom house with its orange door.

A handoff should feel clear from the start.

  1. Submit the referral. Share the basics: who needs support, where they live, what service is being considered and how to reach you.
  2. We confirm receipt and review it. You receive confirmation that we received the referral. A coordinator will follow up with next steps.
  3. We connect with the family. A coordinator listens, explains the options that fit and arranges language assistance when needed.
  4. We work through the requirements. We help the family understand eligibility, authorization, assessments and paperwork, and coordinate with you where needed.
  5. We keep you informed. Your Freedom contact stays available for questions and updates as the referral moves forward.

What a referral to Freedom looks like from your side

  • One owner

A named Freedom contact who knows the referral and can tell you where things stand.

  • A clearer handoff

We work through program and authorization questions with the family instead of sending them back to you to sort out alone.

  • Support that respects the person

Language assistance is available, and our teams use what families tell us about routines, culture and preferences when planning care.

  • Oversight behind the care

For applicable agency services, Freedom-employed caregivers are W-2 employees. Visits are verified electronically where required, with compliance oversight behind service delivery and billing.

Services families may need

Freedom provides home care and attendant care, adult day health, Intellectual and Developmental Disability Programs and veteran care in the communities where those services are available. Freedom also provides conflict-free Kentucky waiver case management.

A human handoff matters.

A referral is more than a form. The family should know who is calling, what happens next and who to contact if something changes. We want families and referral partners to know what happens next.

Know someone who needs care?

Send the referral through the referral form or call us. We will review the request and connect you with the right Freedom team.

Online requests are not available yet. Please call 502-FREEDOM (502-373-3366) and a coordinator will help.
